New Patient Registration

Our policy is not to accept a new patient who lives outside a 7 mile radius of Moneymore. If you do live or have recently moved within our practice area, you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You will need to complete both a registration form and a health questionnaire which provides useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are required to attend a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Change of Personal Details

It is very important to let us know of any change of name, address or phone number in order to enable your records to be kept up to date.  If you leave the area, it may be necessary to register with another practice.
